Enrollment Check List
- Check or Money Order for enrollment fee ($25 per child)
- One enrollment package must be filled out per child
- Bring copy of shot records
- Bring copy of lunch status form
- Pre-K wellness form filled out by doctor
- Emergency contact and alternative pickup persons complete information (i.e. phone number, address)
- Contact information for your child/children doctor and hospital (i.e. phone number, address)
NOTE: If you owe any outstanding balances for LINC child care from previous years, you will not be able to enroll your child/children for the 2011-2012 school year.
